Stolen phone : how to block it so the thiefs cannot use it?

My phone was stolen.
I want to block it in order that whoever has it cannot use it, and will not have the possibility to use it in the future.
I have the IMEI and proof of purchase and statement from the police that was stolen.

How can I request it?
